Uptown Long Beach Youth Baseball has established the following guidelines and reporting forms for such incidents.

_ PLAYER MEDICAL RELEASE & SAFETY FORMS _
Player safety is of the utmost important. But on those occasions where a player may become injured,
the proper treatment and treatment facility must be given to the player. Therefore, Uptown
Long Beach Youth Baseball utilizes the following forms that are carried by all teams to authorize the medical
treatment of any player that is injured in the event that the parent or guardian cannot be reached.

2018 SAFETY IMPROVEMENTS
As part of UPTOWN Long Beach Youth Baseball dedication to safety, we are continually improving our fields.
As part of our safety initiative, we conduct field needs surveys every year and then set aside part of our budget to make improvements.
2018 identified the following needs:
1. Field 2 - New Dugout Benches = COMPLETED
2. Field 1 - New Bull Pen Pitching Area
3. Field 2 - New Bull Pen Pitching Area
4. Field 1 - Overhead Protective Cover For Bleachers
5. Field 1 - New Dugout Wraps = COMPLETED
6. Field 3 - New Storage Bins And Security For Dugouts = COMPLETED
